You have probably heard the rule: for every cup of coffee, drink a glass of water, because the coffee is pulling fluid straight back out of you. The idea is that caffeine makes you pee more, so a coffee habit leaves you short on fluid.

It sounds mechanical enough to be true. A controlled trial says it mostly isn’t.

A quick note on what this is: we are not doctors or physiologists, and this is a piece of reading and reflection on the research, not medical advice. The main study here is a single controlled trial in a specific group of people, and population-level findings are not a prescription for any one reader’s body or health.

Where the diuretic reputation came from

The dehydration story isn’t invented out of nothing. It rests on real caffeine research. A 2014 Birmingham paper notes that “When consumed in large doses (≥500 mg), caffeine elicits a diuretic effect”. But that threshold matters. A typical cup of coffee contains much less caffeine than that.

Much of the earlier research discussed by the authors used pure caffeine or relatively high doses, sometimes in people who didn’t regularly consume caffeine or had abstained beforehand. Pointing back to work by Eddy and Downs in 1928, the authors write that “regular caffeine consumption may lead to a tolerance developing against its diuretic effect”. In plain terms: your body may become less responsive to that effect with regular use. The paper also notes that this tolerance can be reduced after only a few days without caffeine.

What the Birmingham trial did

The researchers tested coffee itself in regular coffee drinkers. Killer, Blannin and Jeukendrup at the University of Birmingham studied 50 habitual male coffee drinkers who normally consumed three to six cups a day.

Every participant did both conditions, so each person acted as his own comparison. Each man consumed four 200 mL mugs of coffee a day during the coffee trial and the same volume of plain water during the water trial, with a ten-day gap between them.

Average caffeine intake during the coffee condition was about 308 mg a day. The researchers measured total body water using deuterium oxide, a stable isotope tracer, alongside urine, blood and body-weight measurements.

What came back

The two conditions were, for practical purposes, the same.

Total body water came in at 51.5 kg on coffee versus 51.4 kg on water, with no statistically significant difference. Urine over 24 hours was almost identical too, 2409 mL against 2428 mL

Body weight fell by about 0.4 kg over the trials, but it fell in both the coffee and water conditions. The authors calculated the daily decline at around 0.2% of body mass and noted that this remained well below the 1 to 3% loss associated with clinical dehydration. One measurable difference was that participants excreted slightly more sodium in their urine during the coffee condition.

The authors’ bottom line is measured: “coffee, when consumed in moderation by caffeine habituated males contributes to daily fluid requirement and does not pose a detrimental effect to fluid balance”. For these men, coffee counted toward their fluid intake and didn’t leave them dehydrated.

It’s also fair to flag that the study was funded by the Institute for Scientific Information on Coffee, an industry-backed organization. The paper states that the funder had no role in the study design, data collection, analysis, publication decision or preparation of the manuscript.

This is one trial, in 50 healthy men who already drank coffee regularly, at a moderate caffeine dose. It does not show that caffeine can never increase urine output.

A separate meta-analysis by Zhang and colleagues found that caffeine produced a small increase in urine output at rest but that the effect disappeared during exercise. Across the included studies, caffeine increased urine volume by about 109 mL on average. The analysis also found a larger diuretic effect in women than men, another reason not to assume the Birmingham study applies identically to everyone.

How I’d read this for an ordinary coffee habit

For a regular drinker having a few cups spread through the day, the fair read is that coffee contributes to hydration rather than simply cancelling itself out.

Caffeine can increase urine production, particularly at higher doses or in people who are not accustomed to it, but that is not the same thing as saying an ordinary coffee habit causes dehydration.
